How do I elect to share my information with another person?

Due to regulations we are unable to disclose your personal health information to outside parties without your written consent.  If you would like to share any personal information with outside parties you can fill out our request for authorization to disclose PHI form.  This will allow us to share PHI information as well as discuss your prescription history with the designated person.
